Quick answer: a semaglutide dosage chart always starts low and steps up. Ozempic and Wegovy injections start at 0.25 mg once a week and rise every 4 weeks, to 0.5-2 mg for Ozempic and 1.7 or 2.4 mg for Wegovy (up to 7.2 mg in some adults). The Wegovy pill starts at 1.5 mg a day and reaches 25 mg on day 91.[1][2] Rybelsus goes from 3 mg to 7 mg and then 14 mg daily.[3]

The tables below come straight from the current prescribing information. Your clinician sets your actual dose and may move more slowly. For background on the drug itself, see our semaglutide profile and the semaglutide topic hub.

Ozempic injection dosage chart

Ozempic is approved for type 2 diabetes, for lowering heart risk in adults with type 2 diabetes and heart disease, and for protecting the kidneys in adults with type 2 diabetes and chronic kidney disease.[1]

WeeksOnce-weekly doseWhat the label says
1-40.25 mgStarting dose
5 onward0.5 mgFirst maintenance option
After at least 4 weeks on 0.5 mg1 mgIf more blood sugar control is needed
After at least 4 weeks on 1 mg2 mgMaximum dose

For chronic kidney disease, the label sets the maintenance dose at 1 mg weekly, reached after at least 4 weeks on 0.5 mg.[1]

Ozempic pen strengths and doses per pen

Ozempic pens are multi-dose, dial-a-dose pens. Each one holds 3 mL, and the concentration differs by pen.[1]

Pen (total strength)ConcentrationDoses it can giveDoses per pen
2 mg/3 mL0.68 mg/mL0.25 mg or 0.5 mg4 × 0.25 mg plus 2 × 0.5 mg, or 4 × 0.5 mg
4 mg/3 mL1.34 mg/mL1 mg only4 × 1 mg
8 mg/3 mL2.68 mg/mL2 mg only4 × 2 mg

The 2026 label also lists single-dose prefilled syringes of 0.25 mg, 0.5 mg and 1 mg, sold in boxes of four.[1] To work out when a pen is finished, see how to tell when an Ozempic pen is empty.

What about “mg per click”?

The pen clicks as you turn the dose selector, but the instructions say not to set a dose by counting clicks. You turn the selector until the dose counter lines up with your dose. The pen only stops at the doses it is designed for, and it stops early when there isn’t enough left for a full dose.[1] Click-counting to get in-between doses is not part of the label. Our microdosing GLP-1 guide explains what evidence exists for that.

Wegovy injection dosage chart

Wegovy injection uses one escalation schedule for every indication: weight reduction, heart risk reduction, teens aged 12 and older, and the liver condition MASH.[2]

WeeksOnce-weekly dose
1-40.25 mg
5-80.5 mg
9-121 mg
13-161.7 mg
17 onward2.4 mg (recommended) or 1.7 mg

If a step causes problems, the label suggests waiting 4 more weeks before the next increase.[2] For adult weight reduction, people who have tolerated 2.4 mg for at least 4 weeks and need more weight loss can go up to a maximum of 7.2 mg weekly.[2] Novo Nordisk announced the approval of this high-dose version, Wegovy HD, on March 19, 2026.[7] In the STEP UP trial, 7.2 mg produced an average weight loss of 18.7% at 72 weeks, compared with 15.6% on 2.4 mg.[5] Whether starting higher than 0.25 mg is ever allowed is covered in can you start Wegovy at 1.7 mg.

Wegovy pill (oral semaglutide 25 mg) dosage chart

The Wegovy pill was approved on December 22, 2025, as the first oral GLP-1 for weight management.[4] It is taken once daily.

DaysOnce-daily tablet
1-301.5 mg
31-604 mg
61-909 mg
91 onward25 mg (maintenance)

The 25 mg dose is the maintenance dose for both weight reduction and heart risk reduction. If someone can’t tolerate it, the label suggests switching to Wegovy injection 1.7 mg weekly.[2] In the OASIS 4 trial, people taking 25 mg daily lost 13.6% of their body weight on average at 64 weeks, compared with 2.2% on placebo.[6]

The pill only works if you take it the right way: on an empty stomach in the morning, with no more than 4 ounces of water, then nothing else for 30 minutes.[2] Our guide to the best time to take a GLP-1 covers these rules.

Rybelsus and Ozempic tablets dosage chart

Rybelsus is the older oral semaglutide for type 2 diabetes. The label now also covers a newer formulation sold as Ozempic tablets. The two use different strengths and are not interchangeable milligram for milligram.[3]

DaysRybelsusOzempic tablets
1-303 mg (starter dose, not effective for blood sugar)1.5 mg (starter dose, not effective for blood sugar)
31-607 mg4 mg
61 onward7 mg, or 14 mg if more control is needed4 mg, or 9 mg if more control is needed

After the first 30 days, people can switch between them: Rybelsus 7 mg matches Ozempic tablets 4 mg, and Rybelsus 14 mg matches Ozempic tablets 9 mg.[3]

Switching between injection and pill

The labels give specific switch points.

  • Ozempic 0.5 mg injection to tablets: one week after the last injection, start Rybelsus 7 or 14 mg, or Ozempic tablets 4 or 9 mg.[3]
  • Wegovy 2.4 mg injection to Wegovy pill: one week after the last injection, start 25 mg daily.[2]
  • Wegovy pill to Wegovy injection: the day after the last 25 mg tablet, start 2.4 mg weekly (or 1.7 mg if the pill wasn’t tolerated).[2]

Why every schedule starts low

All of these schedules increase slowly to reduce nausea, vomiting and diarrhea, which are the most common reasons people stop the drug.[1][2] The starting doses are mainly there to get your body used to the drug, and the Rybelsus and Ozempic tablet labels say so directly.[3] If you miss doses along the way, the restart rules differ by product. See what to do about a missed GLP-1 dose.
