Quick answer: does BCBS cover Wegovy? It depends on which Blue plan you have. Blue Cross Blue Shield is a federation of independent companies, and coverage differs widely.[1] The Federal Employee Program covers Wegovy with prior approval at a BMI of 30, or 27 with a related condition.[2] Blue Cross Blue Shield of Massachusetts excluded GLP-1s for obesity for most members starting January 1, 2026, and Blue Cross Blue Shield of Michigan cut weight-loss coverage for large fully insured groups in 2025.[3][4]
Prices and program terms checked October 2026. They change often; confirm with the manufacturer or your plan.
Wegovy contains semaglutide and is approved for weight management, heart risk reduction in people with heart disease and, as an injection, MASH with moderate to advanced liver fibrosis.[6] For an overview across insurers, see does insurance cover Wegovy.
Why Blue plans differ so much
The Blue Cross Blue Shield Association is made up of more than 30 independent, locally operated companies.[1] Each sets its own drug lists. Many large employers also self-fund their plans and decide for themselves whether to cover weight-loss drugs. Nationally, 19% of large firms offering health benefits covered GLP-1s for weight loss in 2025.[11]

Federal Employee Program (FEP Blue)
FEP Blue’s policy for Saxenda and Wegovy, effective February 13, 2026, covers Wegovy injection for chronic weight management with prior approval. Adults need:[2]
- a BMI of 30 or higher, or
- a BMI of 27 or higher plus either established heart disease (such as a past heart attack, stroke, peripheral artery disease or a coronary procedure) or a weight-related condition such as type 2 diabetes, high cholesterol or high blood pressure
- participation in a comprehensive weight management program
- no use of another GLP-1 at the same time
Children 12 to 17 qualify with a BMI at or above the 95th percentile for their age. For renewal, adults must lose at least 5% of their starting weight or maintain that loss. FEP Blue also has separate criteria for Wegovy tablets and for Wegovy injection in MASH.[2]

Blue Cross Blue Shield of Michigan
In 2024, BCBSM told providers it would tighten prior authorization for weight-loss GLP-1s (Wegovy, Zepbound and Saxenda) from August 1, 2024, and end that coverage for large group fully insured commercial plans on January 1, 2025. Diabetes GLP-1s stayed covered.[4] A May 29, 2025 revision of its member article describes prior authorization for those drugs at a BMI of 35 or higher for adult members of those plans.[5] If you are a BCBSM member, ask how this applies to your group.
Coverage by use
Even when weight-loss drugs are excluded, a plan may still cover Wegovy for a disease it treats:
- Heart risk reduction in people with established heart disease and overweight or obesity.[6]
- MASH with stage F2 to F3 fibrosis, as an injection.[6] FEP Blue has separate MASH criteria.[2]
Ask your plan specifically about these uses if they apply to you.
Blue Medicare and Medicaid plans
Medicare. If you have a Blue Medicare Advantage or Part D plan, Wegovy for weight loss is available to eligible people for $50 a month through the Medicare GLP-1 Bridge from July 1, 2026 to December 31, 2027.[7] Prescriptions to lower cardiovascular risk go to your Part D plan instead.[8] See does Medicare cover Ozempic.
Medicaid. Blue Medicaid plans follow state rules. 13 state Medicaid programs covered GLP-1s for obesity as of January 2026.[9] See does Medicaid cover Ozempic.
How to check your Blue plan
- Find your Blue company on your ID card, or use the BCBS Plan Finder.[12]
- Search your formulary for “Wegovy.” “Excluded” or “not covered” means a benefit exclusion; “PA” means coverage with prior authorization.
- Ask HR whether your plan is self-funded and whether weight-loss coverage or a rider is included, and whether anything changes at renewal.
- Find the policy. Many Blue companies post their GLP-1 policies, as FEP Blue does.[2]
- Have your prescriber submit PA with your BMI, conditions and lifestyle program records. Our Wegovy prior authorization guide explains how.
Questions to ask your Blue plan
When you call member services, these questions get to the answer quickly:
- Does my plan cover drugs for weight loss, or are they excluded?
- If they are covered, which policy applies, and what BMI and program requirements does it set?
- Is Wegovy covered for heart risk reduction or MASH, even if weight-loss drugs are excluded?
- Is the Wegovy pill covered, or only the injection?
- Does my coverage change at my employer’s next renewal date?
- If I already have an approval, when does it expire?
Write down the name of the person you spoke with, the date and any reference number. If you later appeal, these notes help show what the plan told you.
Paying less
If your Blue commercial plan covers Wegovy, Novo Nordisk’s savings offer can cut your cost to as little as $25, with up to $100 off a one-month fill. It cannot be used with Medicare, Medicaid or other government plans.[10] If your plan excludes Wegovy, compare cash prices in how to get semaglutide without insurance. If you have type 2 diabetes, see does BCBS cover Ozempic.
